Family Survey

Dear Parents, It is very important to complete your Family Survey that was sent to your home on November 7. The funding provided to Mount de Sales Academy in the way of book bill credits, technology funds and professional development programs will be impacted directly by your response. The NEW CALCULATION of funds will be determined by the percentage of responses we have received from our total enrollment. For example, the $65 credit on each student’s book bill will be reduced to $48.75 if only 75% of our families respond. Please take the time to complete the survey and return it to the school via the homeroom teachers. If you have misplaced the survey form, please contact the business office and we will be happy to send another. We would like to take advantage of 100% of outside funding to help defray the cost of tuition for our students.
